I have the opportunity to be part of the infection control quality for the past 8months and I’ve come across numbers of different infections from MRSA to CDiff yet more than 60% of the infections comes from UTI (Urinary Tract Infections). The main focus of my facility was to build an infection health program that will help drastically decrease the use of improper antibiotics. After this program was implemented, it was a tremendous success within the facility with prove of infection rates being reduced. All 28 facilities will be presented with this program and its focus will be to decrease UTI by implementing our tool and eradicating unnecessary procedures. Who is your target population?

 

My target population are geriatric patients in a long-term facility because studies have shown that residents who stay in a long-term facility have a higher risk case of acquiring UTI.

The role of the Registered Nurse in the health care program is to implement the (SBAR) Situation, Background, Assessment and Request for suspected UTI oppose to just randomly asking doctors for urinalysis. Nurses will play and essential role by collaborating with other health professionals by making sure that patients get the appropriate antibiotic he/ she needs. What is your role as an advocate for your target population for this healthcare program?

 

My role as an advocate is to target the geriatric population living in nursing homes because long-term facilities have the most common antibiotic resistant strain which can sometimes be deadly to older people with weakened immune system. My focus will be to get rid of unnecessary antibiotics that residents don’t need which may lead to antibiotic resistance and get them much needed appropriate treatment.

 

Do you have input into design decisions? How else do you impact design?

 

Each suspected tool kit form will be addressed by me and will be discussed with other health care team for appropriate treatment and follow-up. As the infection control nurse, I will make sure to follow up with every suspected UTI case by assessing each resident and following up with each case to rule out if it’s true UTI or underlined disease before contacting the physician.

 

What is the role of the nurse in healthcare program implementation?

 

The Rn role in the health care program implementation is to come up with a program which focuses on getting patients off unnecessary antibiotics which can become resistant. In other to get to these conclusions the RN will play an important role by working alongside with other health professionals to make sure that residents get the right antibiotic. Who are the members of a healthcare team that you believe are most needed to implement a program? Can you explain why?

Nurse Practitioners/ Doctors are the main health care members mostly needed because they are the ones responsible in prescribing the antibiotics. Periodically a letter will be addressed and sent to every member alerting them about the clinical guidelines developed with the nursing homes. The nursing facility will modify its protocol around urine analysis to improve the use of antibiotic for the treatment of UTI, also will develop an SBAR toolkit that all nurses have to follow before informing any clinician. The SBAR toolkit will be research based and will be used as a method of communication between nurses and practitioners to get important patient information. Other Members who will be involved are the DON (Director of Nursing), RN, LPN and Nursing assistants.

In summary, Strong leadership is important in the goal of transforming the healthcare system. In the medical field; nurses must assume the role of a leader and develop health care programs.
